چکیده
The endoscopic resection of colorectal polyps that are limited to the mucosa is a well established procedure. Nevertheless, modern imaging technology and screening programs have increased the detection of benign but large (>20mm) polyps. In flat laterally spreading tumors (LST), especially the nongranular (NG) type, the rate of submucosal invasion is found to be as high as 39% (34%–44%) [1]. For this reason a resection in one piece is mandatory [2]. We report on the resection of a 25-mm flat rectal LST-NG in a60-year-oldpatientwho was admitted to the Klinikum Augsburg after screening colonoscopy (●" Fig.1). Examination with a high definition colonoscope (CF-HQ 190I; Olympus Medical Systems, Tokyo, Japan) demonstrated a SanoType II, pit pattern IIIs lesion. We decided to use two newly developed devices, the Flat Adenoma Resection Instruments (Endox-Feinmechanik GmbH, Bad Urach, Germany). Submucosal injection was performed using a mixture of saline, epinephrine (1:100000), glycerol

Probe-based confocal laser endomicroscopy (pCLE) images of submucosal growth of a duodenal mucous neck cell adenoma.
Probe-based confocal laser endomicroscopy (pCLE) is an imaging technique that provides in vivo and real-time endomicroscopic analysis [1,2]. We show images of a duodenal mucous neck cell adenoma growing in the submucosa that was visualized by pCLE. A77-year-oldmanpresentedwith a3-mm protruding lesion in the duodenal blub (●" Fig.1). The lesionwas located on top of a submucosal elevation of appr...
متن کاملJGES guidelines for colorectal endoscopic submucosal dissection/endoscopic mucosal resection.
Colorectal endoscopic submucosal dissection (ESD) has become common in recent years. Suitable lesions for endoscopic treatment include not only early colorectal carcinomas but also many types of precarcinomatous adenomas. It is important to establish practical guidelines in which the preoperative diagnosis of colorectal neoplasia and the selection of endoscopic treatment procedures are properly...
متن کاملSuccessful endoscopic submucosal dissection for triple sporadic nonampullary duodenal adenomas using a "push and peel off" technique.
Sporadic nonampullary duodenal adenoma (SNDA) is not easy to find in symptomless patients. SNDA has low incidence and usually occurs singly [1]. Resection of duodenal neoplasm by endoscopic submucosal dissection (ESD) has been increasingly reported [2]; however, resection of triple SNDAs has not been reported before, to our knowledge. Here we describe a case of successful ESD for triple SNDAs u...
متن کاملPreventing and Controlling Bleeding in Gastric Endoscopic Submucosal Dissection
Although techniques and instruments for endoscopic submucosal dissection (ESD) have improved, bleeding is still the most common complication. Minimizing the occurrence of bleeding is important because blood can interfere with subsequent procedures. Generally, ESD-related bleeding can be divided into intraprocedural and postprocedural bleedings. Postprocedural bleeding can be further classified ...
متن کاملComparison of O-Type HybridKnife to Conventional Knife in Endoscopic Submucosal Dissection for Gastric Mucosal Lesions
Endoscopic submucosal dissection (ESD) has been accepted as a minimal invasive alternative to surgery for localized superficial gastrointestinal neoplasms recently. However, the procedure remains to be technically challenging and time consuming. A new dissecting knife with partially insulated tip has been recently developed with built-in injection capability. The purpose of this study was to in...
